I really like Slayers. I don't know what about it exactly that makes me love it so much, but its probably my favorite.
For manga I really like Uzumaki and about everything else Junji Ito does.

It's been a while, and I haven't read through all of either, but I especially like the mangas Black Jack and COWA!
I haven't watched anime in a while either. I found myself liking Okami-san & Her Seven Companions despite it being really rock stupid.

I'm going on record here and saying that Bleach is a derivative schlock fest and I gave up all hope for it once Grand Fisher, THE BEST VILLAIN IN THE WHOLE SERIES WHO TIED INTO THE PROTAGONISTS BACKSTORY PERFECTLY, was killed off handedly in two frames to make Ichigo's dad seem like a badass but then he never did anything again.
But I LOVE One Piece. It's got a great protagonist, story, character, and Oda's art is fantastic. It really lends to the wackiness of the world but also the darkness of scenes like where a guy has to eat his own leg. It's also the most plotted out of the big 3. Easy to see why its the best selling comic in the world right now.
Naruto had a promising year or two during its beginning but it eventually collapsed under its own weight. I think it started going downhill when the built up to Rock Lees return for three sagas only to have him get drunk and let Gaara save him, the guy who nearly killed him in the first place. If the show really wanted to make a dramatic scene, it should have been the other way around. Would have contrasted nicely with Shikamaru and Kiba getting saved. I think we all know how it goes after the time skip.
Past the big three I like Welcome to the NHK, Baccano, Ghost in the Shell 1st Gig, and Paranoia Agent. I may also be reading a certain manga about a high school student who saves the world with his knowledge of visual novel gal games as a extremely guilty pleasure. For a series with that premise, I was shocked with how much effort went into it.

Uzamaki is good but I have to wonder once the second volume starts WHY THE HELL SHE DOESN'T LEAVE THAT FRIGGIN TOWN ALREADY.

QFT!!!
Bleach eps. 1-10 was what made me fall in love for that series. Granted the Reaper Society arc was pure shonen, but at least it was constructed well and the momentum of the beginning pushed the story through it well enough. But after that, the series might as well have ended right there.
Grand Fisher SO should have somehow gained ginormous power and become the arch nemesis of the protag. Note, I don't think that would make a good story like eps. 1-10 was. But if the author absolutely wants drag-on shonen, that would have been the better way to do it. Stories where the villain develops along with the protag, are better than ones where the villain is static. And he would have been like f'ing Kefka!

I love Deadman Wonderland manga-wise, though I've not met many people who've heard of it, let alone read it.

I liked Bleach, until they had the main character go to that other land place all the other reapers and then I got bored. But I loved it up until that point.

So I've currently finished watching Darker Than Black season 1.5, and thru 4 eps of season 2.
I've watched DTB season 1 a long time ago, and really liked it, but didn't continue because somebody complained that the Protag acts really OOC in season 2.
Now that I've watched 1/3 of season 2, I have to disagree: I feel the Protag is quite in-character. I am enjoying seasons 1.5 and 2 very much.
The only nitpick may be that it's cliche for every hero going through a personal crisis to turn to alcohol. But in this case there is actually a good reason if you think about it: This Protag's superpower requires him to eat a lot of calories for him to keep himself "refueled." In the past he just ate 10 people's worth of meals. Now he drinks alcohol constantly to do it. He does get drunk (somewhat), but he's also refueled.

I don't know if anyone said this yet (didnt read all the replies, dont have time to) but Full Metal Alchemist. :D I love the art, I love the story, I love the meaning behind it.
